*Due to COVID-19, I'm offering video or phone consultations rather than In-Home appointments until further notice. Let's review your health plan needs and all of your options. Have a list of physicians, current medications, and any coverage concerns handy and I will walk out through all the options so that you can make the best-informed decision for you and your family. Please specify phone or video via Zoom consultation, so that I can send you the appropriate video link to confirm, if applicable. If no specifications, I will simply default to a phone call to the number listed on the appointment. You will be sent an email with a Scope of Appointment form that Medicare requires to be completed prior to our meeting. Please complete and submit at your earliest convenience.
